Questionnaire Quote Form (QQF)

1)  Bride submits our questionnaire quote form.  This lets us know more about you and your style so
we can tailor our flowers to your needs and wants.  Please Answer all the questions.  We can only go
by what you tell us.

Free initial consultation

2)  One of our design specialists will contact you by email or phone to go over your form.   Please feel
free to use this time to ask any questions you may have.
*Please note*:  If you haven't received an email from us in 3 days, please check your bulk email folder.

Contract

3)  Once you have decided on your flower box details, Read over the contract in the link we provided.  
This contract will also need to be printed and sent to us via mail or fax.  The corresponding deposit will
need to be sent by cashier's check, money order, visa or mastercard.  *Please note* that your date is
not reserved until contract AND payment has been received.

Credit Card User Note

Clients paying by credit card will be charged a 3% convenience fee.  Also please note the credit card
machine we are using is the mother of Rai (the owner and lead designer) machine from her previous
business named Hollywood ladies apparel.  Since we are great designers but technology challenged
we haven't figured out how to change the name.  So when Hollywood ladies apparel is on your
statement, don't worry.  That's us!

Pictures

At this time if you have any pictures you would like us to closely replicate or design styles you like
please submit them through email.

Final payment

40 days prior to your wedding, your final payment is due.  Other an a friendly reminder 5-10 days prior,
we will not be contacting you in regards to final payment.  We don't like to be bill collectors.  Its not fun
for us or you.  It is your responsibility to make your payment on or before the due date.  *Please note*:
Design work does not begin until package is paid in full.  Late payments will delay your shipping.  The
client will have to pay for rush shipping charges due to late final payments.  There is a $35 late fee for
any late final payments.  Final payment due date is on your contract.

Final Consultation

25-30 days before your wedding date you are contacted again for a final consultation by phone or
email.  *Please Note*: we will attempt contact 3 times.  If we haven't been able to contact you, we will
then assume no changes of any kind has been made and continue accordingly.  We go through your
order details.  If you need to make any additions and haven't yet the deadline is now.  We also give
you an approximate date your order is being shipped.  *Note*: If you haven't paid the remaining
balance of your order, this date (by credit card or cashier's check/money order rushed) is the absolute
deadline.  (If we receive payment after this date your order will be delayed.)  At this time you will be
emailed a photo for your approval prior to shipment.  If needed (which rarely happens) we will provide
1 rearrange free of charge.  Each subsequent change is $40.00.

Shipping

Please allow 2 weeks for delivery.  Rush orders can be processed.  Please call or email
rush requests  *Please note* HeartFelt Creations is NOT responsible for lost, damaged, or stolen
packages through delivery service.

Cancellations and Returns

Cancellations are accepted up to 40 days prior to event date submitted.  Please see contract on
cancellation terms.  Returns cannot be accepted since each arrangement is handcrafted and custom
processed uniquely for each client.

What comes in my box?

Packing slip, your QQF, Invoice, Photos of arrangements, and your beautiful flowers!
